This book describes the application of major safety reviews used in the process industries, with a
focus on the petroleum, petrochemical, and chemical sectors. The underlying principles and practices
can readily be transferred to any operation that involves a continuous or batch process system which
is subject to a major incident with the potential for injuries, fatalities, major property damage,
environmental impact, or serious effects on business operations. The book provides guidance on
qualitative hazard analyses for review teams. OSHA and EPA, as well as national governments all over the
world, require industry to conduct these reviews as a safeguard against catastrophic fire, explosions, and
oil spillages.

Industries and was issued in 1994. Since that time, the use of Process Hazard Analyses (PHA)s has become more prevalent and the threat to
industrial and commercial facilities from security incidents has also become more relevant..It was
therefore felt prudent to update the first edition to include these aspects and also incorporate additional
technical updates and features...”
